When you turn 18, you have the right to choose where you want to live, even if you have a disability. Moving out of your family home and into your own place can be a big decision, and it's normal to feel excited and nervous.
The Transition Team is here to support you when you're ready to explore independent living. We'll work with you, your caregivers, and partners such as the local borough council housing service to find suitable care, support, and accommodation.
