How we work with you

This page is about how the Transition Team works with you. We want to hear your views and help you plan for your future.

If you have an Education, Health and Social Care Plan (EHCP) review at school, the Transition Team may come along from Year 9.

We can give you advice and start to think about the support you might need when you turn 18.

Your voice

The Transition Team wants to hear your views because it's your life. We will follow the Mental Capacity Act. It says that every adult with a disability has the right to make their own decisions, wherever possible.

If you need help sharing your views, you can get an advocate. An advocate helps you speak for yourself and makes sure your voice is heard.

You can call or email the Transition Team to speak to someone about your views.
