The following people are involved at this step:
- an occupational therapist (OT) or OT assistant (OTA)
- you
They will carry out your assessment. They'll talk to you about what you can manage to do at home now and what you need help with. This will help them to decide if you need:
- advice
- signposting or referring you to other services
- providing you with equipment
- installing minor and / or major adaptations
If you need major adaptations, the assessment will include:
- whether you might get a Disability Facilities Grant (DFG)
- the Occupational Therapy Major Adaptation Report (OTMAR)
- advice on what adaptations will meet your long-term needs
- draft plans of how to change your home to meet your needs
You will need to:
- sign your OTMAR
