What happens next

After you send us your form, we'll email you to confirm we have it. Keep this email for your records. If you don't see the email, check your junk folder. If you're still having trouble, contact us.

We will check to make sure we have everything we need before sending your information to the right local care team. They will contact you to set up a meeting to discuss what you've told us in more detail. This might be face-to-face, virtual or over the telephone.

After your meeting we'll send you a letter to let you know our eligibility decision. This will include a copy of your assessment. If you have eligible needs, we'll include the actions agreed with you to meet some, or all, of your needs. You will then be asked to do a financial assessment.

If you do not have eligible needs, we will give you information and advice about what care and support is available to help you in the community. For example, this could be from local charities or voluntary organisations. If any children in the household have caring responsibilities, we will also provide you with information available to them as young carers.
