A Stroke Coordinator works with you to develop a personalised plan. You'll identify the actions needed to reach your agreed desired outcomes. Support begins following a stroke and continues on a needs led basis.
The service is for Surrey adults affected by stroke. This includes stroke survivors and their unpaid carers aged 18 and above.
You can choose to get support face-to-face or virtually. Support Coordinators can also meet with individuals within hospital settings.
