Director of Prevention and Home First

Adult Social Care and Health services in Surrey are good but we want them to be better. We know where we are coming from and we know where we are going. We have recently bought Adult Social Care, Public Health, and Community and Health Partnerships functions together into one directorate to ensure we can continue to deliver our core principle of supporting people to live their best life. To meet the demands and challenges of tomorrow, we are building our leadership capacity today. Our new structure in Adults, Wellbeing and Health Partnerships will increase the focus on quality and good outcomes; lead a productive and motivated workforce; support inclusive communities to reduce inequalities; and ensure we continue to deliver our statutory duties in a financially sustainable way.

As our Director of Prevention and Home First, you will provide strategic leadership to advance our Integrated Home First model and champion initiatives that manage demand and maximise the use of technology-enabled care. Your role will be pivotal in shaping the future of adult social care, ensuring efficient operations and delivering services that significantly enhance welfare and overall wellbeing.

We are seeking a proactive individual to lead the development and implementation of prevention and early intervention initiatives, fostering a culture of high performance and continuous improvement. Your role will involve leading the Integrated Home First model across all acute hospitals, aimed at maximising independence and improving outcomes for individuals and their families. As a champion of the adult social care Front Door, you will ensure efficient access to social care services that are responsive to individual needs. Additionally, you will lead initiatives utilising technology in care, empowering individuals to take control of their health, wellbeing, and support.
