The starting salary for this full-time, permanent position is £35,730 per annum based on a 36-hour working week.

We have an exciting opportunity for a Personal Advisor to join our Children with Disabilities Service, based in Woking. This role is open to hybrid working meaning you aren't required in the office 5 days a week. We'd welcome a conversation with the successful candidate on what hybrid working may look like for you.

Please note, the contractual location for this role is currently Quadrant Court, however it is our intention to move within the local area to Victoria Gate in Central Woking later this year.

About the Team
The Children with Disabilities Team purpose is to work with and provide support to children and young people who have severe and profound disabilities. Some of the children and young people are Looked After and we operate an 18 – 25 years service to those who have ceased to be looked after by the local authority and who are eligible for a service as per the Children Leaving Care Act 2000 and/or The Children and Social Work Act 2017. Our vision is to support eligible disabled young people to achieve their potential as they make their transition to adulthood. We work closely with Transition colleagues in Adult Social Care and with colleagues in Health to support early planning for independence and appropriate accommodation when young people reach 18.

About the Role
As a Personal Advisor you will case hold young people with severe and profound disabilities with complex needs and care arrangements. You will participate in the assessment and preparation of Pathway Plans for those who are eligible. The Pathway Plan sets out the young person's aspirations regarding education, accommodation, health, social relationships and specific needs. You will work across CWD and the Transition team to ensure that young people are supported to be as independent as possible and to ensure that their care and support needs are addressed by adult services.

Working closely with the Team Manager and the wider team, you will make a personal contribution to a collective endeavor, creating the conditions for success and actively contributing to the development of the service. You will also chair and lead the review of the Pathway Plan according to statutory requirements, including liaising with partners for young people living outside of Surrey.

As a Personal Advisor you will be able to recommend services that the local authority may be required to pay for, and while you would hold no direct budget responsibilities, you would be required to have an understanding of the finance policy. Alongside your caseload, you will be required to carry out duty on a rota basis.

Shortlisting Criteria
To be considered for shortlisting for this position, your application will clearly evidence the following:
  • Ability to develop and sustain good working relationships with young people with complex disabilities and/or health needs and their families
  • A good working knowledge of relevant legislation (The Children Act 1989, Children Leaving Care Act 2004, Care Act)
  • Ability to work to defined reporting standards, ensuring that reports are structured and evidence based
  • An awareness of current issues relating to preparation for adulthood and transition planning
